Surgical consultation

Consultation is required prior to treatment.

Purchase is a pre-payment for services. If your purchased service is not recommended during your consultation, your credit may be used toward any approved service/product. You will be contacted within one business day of purchase to schedule your appointment.

Consultation Booking Fee

I understand this fee is to book a surgical consultation with Dr. Rashid and is not a deposit for surgery.

Nonrefundable Fee

This fee is nonrefundable if I cancel or miss my appointment. If I miss my appointment or reschedule with less than 24 hours notice, a new booking fee will be required to secure a new consultation date.

BMI Limit

I understand that Dr. Rashid has implemented a Body Mass Index Limit (BMI) of less than 38 for major surgeries. I certify that my BMI is 37 or lower.

Address your specific areas of concern with one of our board certified plastic surgeons to cultivate a unique treatment plan. Initial consultations for cosmetic surgery allow our surgeons to understand your goals and provide the education about the procedure. The cost of the consultation is deducted from the cost of surgery when scheduling.
